Thank you for attending our conference. Please complete this evaluation. There are four sections: 1) speaker evaluations, 2) required questions, 3) overall event evaluation, and 4) CME / MOC credit claiming.

After you submit your evaluation, you will receive a confirmation screen with the link for your CME Certificate. Copy and paste into your browser to download our blank certificate. Then fill out your name and number of credits claimed for your records.

If you are requesting MOC points, we encourage you to first complete the evaluation and enter your CME credits claimed. Then complete this required MOC quiz from our speakers. A passing grade of 60% is required to receive MOC points. â€‹

For CME:

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of the Washington State Medical Association (WSMA) and the Endocrine Days Educational Institute. The WSMA is accredited by the ACCME to provide continuing medical education for physicians. 

The WSMA designates this Live activity for a maximum of 6.0 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.


This activity meets the criteria for up to 6.0 hours of Category I CME credit to satisfy the relicensure requirements of the Washington State Medical Quality Assurance Commission. 

 

For MOC:

Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 6.0 MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.
